Despite the recent decline in the teen birth rate, teen pregnancy remains a significantproblem in this country. Most teen pregnancies are unintended.

Despite the recent decline in the teen birth rate, teen pregnancy remains a significant problem in this country. Most teen pregnancies are unintended. Each year, about 200,000 teens aged 17 and younger have children. Their babies are often low birth weight and have disproportionately high infant mortality rates. They are also far more likely to be poor. About 80 percent of the children born to unmarried teenagers who dropped out of high school are poor. In contrast, just 8 percent of children born to married high school graduates aged 20 or older are poor. The U.S. Department of Health and Human Services (HHS) has responded to a call from the President and Congress for a national strategy to prevent out-of-wedlock teen pregnancies and to a directive, under the new welfare law, to assure that at least 25 percent of communities in this country have teen pregnancy prevention programs in place. Building on our previous work in this area, our national strategy is designed to:

Introduction
Despite the recent decline in the teen birth rates, teen pregnancy remains a significant problem in this country. It is a problem that impacts nearly every community. Thus, the responsibility to solve this problem lies with all of us, including families, communities, and young people themselves. The President and Congress called on HHS to develop a National Strategy to address this serious challenge and to assure that at least 25 percent of communities in this country have teen pregnancy prevention programs in placeas mandated under the Personal Responsibility and Work Opportunity Reconciliation Act of 1996. The Department responded to this call by releasing a National Strategy to Prevent Teen Pregnancy in January of 1997. This Strategy presented a comprehensive new plan to prevent teen pregnancies in the United States by strengthening, integrating, and supporting teen pregnancy prevention and other youth-related activities in communities across this country. The Department is required by this law to report to the Congress by June 30

Time to Take a Critical Look at Depo-Provera

What is Depo-Provera?
Depo-Provera is now approved in more than 70 countries including the U.S., France, Germany, Sweden, and the United Kingdom. A once-monthly version of Depo-Provera, called Cyclofem (also known as Cyclo-Provera) has undergone clinical trials in Chile, Indonesia, Jamaica, Mexico, Thailand, and Tunisia. Medical Advertising News predicts strong market potential in Latin American, China and Eastern Europe(2). Who Uses Depo-Provera?: The Image and the Reality
In the United States, despite extensive print and groundbreaking television advertising campaigns targeted toward middle-class women, Depo-Provera is more often utilized by poor or working class women and women receiving primary healthcare from federal and many state-funded health clinics. This is buoyed by an agreement that Upjohn Company made in 1994 to supply Depo-Provera on a "buy one, get one free" basis to Title X-funded agencies and clinics(3). Across the U.S., Depo-Provera has been further subsidized by state and local governments (Houston, Texas recently purchased $1.3 million worth of Depo-Provera for dispensation at city health clinics serving the poorer strata of Houston)(4). In less than a decade, Depo-Provera has had a significant impact. Depo-Provera use has been credited with a dramatic reduction in teen pregnancies and is the contraceptive choice for approximately 8% of white teens and 19% of black teens. The increased media focus on teen pregnancy reduction coupled with the lack of similar focus on the potentially adverse effects of Depo-Provera use are likely to ensure that the numbers of teens on Depo-Provera will continue to rise. A recent study of Massachusetts family planning clinics found that Depo-Provera use has risen 77% over the past three years among women ages 20 and younger(5).

Moreover, because the average age of menarche has reached an all-time low of about 12 or 13 years (Potts, 1990), and because four out of five young people have sex as teenagers (AGI, 1999a), a greater proportion of teenage girls are at risk of becoming pregnant than ever before.
The consequences of adolescent pregnancy and childbearing are serious and numerous:
  • Teen mothers are less likely to graduate from high school and more likely than their peers who delay childbearing to live in poverty and to rely on welfare (Annie E. Casey Foundation, 1998).
    The children of teenage mothers are often born at low birth weight, experience health and developmental problems, and are frequently poor, abused, and/or neglected (Annie E. Casey Foundation, 1998).
    Teenage pregnancy poses a substantial financial burden to society, estimated at $7 billion annually in lost tax revenues, public assistance, child health care, foster care, and involvement with the criminal justice system (Annie E. Casey Foundation, 1998).
As a result, the United States needs a number of initiatives to reduce its teenage pregnancy rate and the negative outcomes that accompany it. These initiatives should incorporate medically accurate sexuality education and information in the schools and in the media, improvements in funding for and access to family planning services, and youth development programs to improve the life options of impoverished teens.

Teen Pregnancy Prevention
For nearly three decades, Family Health Council has been dedicated to the prevention of teen pregnancy in western Pennsylvania. Beginning in 1990, Family Health Council launched a series of specially targeted activities in Allegheny County, including a multi-media campaign; sexuality education guidebooks for parents called Family Connections Teen Connections , a pocket-sized guide for teens; and peer education. The success of these strategies led to the creation of the Center for Adolescent Pregnancy Prevention (CAPP) in 1995. Through CAPP, a regional, comprehensive, multi-disciplinary approach to reducing teen pregnancy in western Pennsylvania was established. Community coalitions of residents, service providers, educators and local leaders work with CAPP to implement programs to meet the needs of their neighborhoods.
