Suicide Crisis Intervention
About 30,000 suicide deaths occur in the United States every year. The number one cause of suicide is untreated depression. Suicide is the culmination of a long process in which people try in various ways to reduce their profound emotional pain. They have contradictory desires to live and to die, and the balance between the two shifts back and forth. Indicators of depression are discussed separately. The following indicators suggest that suicide may be imminent.
  • Pre-Suicide Statements: Direct or indirect statements about suicide, hopelessness or death. Final Arrangements: A person may put their affairs in order, change a will, give away possessions, or talk vaguely of going away. Sudden Elevated Mood: Paradoxically, a depressed suicidal person may suddenly appear better after making a decision to end their life. A burden has been lifted, as they no longer have to agonize over the decision. Risk Taking or Self-Destructive Behavior: This may represent a death wish. The person isn't ready to take their own life but tempts fate by taking an extreme risk, for example reckless driving.
Suicidal people often pick rescuers. Despite what they say, subconsciously they want these people to help them. If you suspect that someone may be contemplating suicide, ask them direct questions. Are you thinking of suicide? How are you going to do it? Do you have a gun, or enough pills for an overdose? When will you do it?

Suicide Crisis Intervention
For most suicidal people, help from others is very desirable for adequate solution of the problems that are drawing them toward suicide. Most potentially helpful are the friends and family of the suicidal person although contact with a professional is essential when the suicide risk is high. For the untrained person, a suicide threat may generate such high anxiety that the problem is dealt with by thoughts of denial in the helping person as well as in the suicidal person. 1) The suicidal person's feelings must not be denied but must be accepted as an important part of the situation. If the person says he feels worthless, it is not useful, and it may be destructive, to reassure him that he is objectively not worthless. This is his subjective reality and therefore a true part of the problem. Similarly, if the person says he wants to die, it is not useful to respond, "Oh, you don't really want to die you're exaggerating." It may be true that death is not his real goal, but he is trying to communicate a true feeling. If another person informs him that what he says is not true, the potential suicide may be confirmed in his notion that no one understands or cares for him. In fact, denial of a feeling may actually be harmful. 2) The suicidal person is confused about what he wants and how to get it. One of the greatest needs is for others to help him clarify his own internal state. Often, this can be done through talking about feelings, whether these are expressed verbally or communicated by gesture or facial expression. The suicidal person may come to understand his own feelings better simply by talking to someone and listening to his own words.
